    Welcome to Knowsley Moving Forward Together

    What we do:

      Knowsley Moving Forward Together has two strands of delivery, both based on the concept of putting you at the centre of the support you received. You can access either, or both support strands depending on your needs and eligibility criteria.

      The social inclusion strand focuses on supporting you to develop your confidence, connect with the community, build up a support network and develop life skills.

      The employment (IPS) strand focuses on supporting you to access employment opportunities. Staff will use their knowledge of person-centred care to empower you to recognise your employability and find work tailored to your aspirations. The team will also work closely with your local Community Mental Health Team to ensure that your goal of gaining employment is part of your care place and that everyone is supporting you to achieve this goal as part of your recovery.

      Who is this service for?

      The service is available for Knowsley residents aged 18+, registered with a Knowsley GP, who are in contact with a mental health service.

      Which part of the service can I access?

      Social Inclusion

      • Open to secondary care or have been discharged in last 6 months.
      • Being supported through primary care (GP or Talking Therapies) and is in regular contact (minimum every 2 weeks) or on a waiting list.

      IPS Employment support

      • Currently being supported by a Community Mental Health Team or Early Intervention Team

      How do I access the service?

        The service can be accessed via self-referral or a referral from a healthcare professional. If you are a professional who would like to make a referral, please download the form here and return it to [email protected].
